How do I create a new consent form?

1-     Select a consent form template from healthcare

2-     Click on "Add New Template".

3-     Enter the name and subject

"If you want to confirm in question format, we will click on the 'Is question text ?

4-     Will write the statement, and if it's a question, mark it as a question and enter the question:

1- Click on 'Add Question' 2- Enter the question 3- Click on 'Submit'

"click save"

- If you want to confirm in article format, do not activate the question format. 1- Write the confirmation in this way

"We will notice that the new consent form templates have been created."

Secondly, how does the patient sign the consent form?

1-    select the consents forms from the healthcare section

2- Click on 'New Consent Form.

3-Choose the patient's name, the doctor's name, and the specific form I want the patient to sign.

4-Choose the form the patient needs to sign

5-Add note if you want, Then click here for the signature, have the patient sign, and then click 'Sign.'"

6-Then click save.

• And that's now the patient signs the consent form.
